Use this form to make a written complaint to the Energy and Water Ombudsman Western Australia. When you send it to us, please include all relevant supporting documentation. Alternatively, you can complete the Online Complaint Form (you will be able to attach electronic documents as supporting information or advise us that you will be sending them in hard copy):
