Exemption Request Form

Oganisations can request an exemption from commencing or continuing an investigation into reportable conduct. This form is not to request an exemption from notifying the Ombudsman of a reportable allegation or reportable conviction. Organisations must still notify the Ombudsman even if they seek an exemption from investigation.

I have formed the view on reasonable grounds that this matter does not constitute reportable conduct.

I have formed the view on reasonable grounds that the report of the matter is frivolous or vexatious or not made in good faith.

I have become aware that another appropriate person or body is dealing with or investigating the matter.

I am required by law to comply with the directions of another person or body in relation to the investigation of the matter.

I have been requested/directed by another appropriate person or body to cease, or discontinue for a period, the investigation of the matter.
Please name the person or body in 3.3 below, and indicate whether they have requested OR directed you to cease or discontinue the investigation.

  • You will not receive a copy of your exemption request after submission. If you would like a copy,  before clicking Submit click Print page Print (you can then use "Print to PDF" or "Save as PDF" to save a copy).
  • You will be provided with a reference number after submission. Kindly record this reference number for your records. If you do not see a submission confirmation and reference number, please contact us to check if your submission has gone through.


Use your own form
